Billing Codes for Custom Made Prescription Orthotics:

Following is a list of currently acceptable insurance billing codes for custom made orthotics and a brief interpretation for your use. Please note, this is only a partial list. For a more complete listing you should refer to the orthotic billing code manual.

L3000

Foot insert, removable, molded to patient model. “UCB” Type, Berkeley Shell, each. Plastic device, molded over model of patient's foot to provide control of the foot.

This code is used for prescription foot orthotics made of rigid or semi-rigid materials for the purpose of controlling the foot by improving foot function. This can include UCBL devices, modified UCB devices and all rigid or semi-rigid FO's made from a model of the of the patient's foot whether plaster, foam or electronic imaging. This code includes all billable fees for the FO including additions such as rearfoot or forefoot posting, padded top covers, soft tissue supplements, balance padding, lesion or structure accommodations and any other additions that may be required.

L3010

Foot insert, removable, molded to patient model, longitudinal and metatarsal support, each. A soft, semi-flexible or rigid device molded over a model of the patient's foot and placed in the shoe to provide support under the longitudinal arch of the foot.

This code is used for a custom prescription leather foot support made on a plaster, foam or electronic image of the foot where no corrections or balancing is performed, providing only longitudinal arch support and no attempt at foot control or improvement of function. Such devices would have a flat heel cup with no heel control, and no posting.

L3020

Foot insert, molded to patient model, longitudinal and metatarsal support, each. A device molded over a model of the patient's foot and placed in the shoe to provide support under the ball of the foot.

This code is for a custom prescription foot support made on a foam, plaster or electronic image mold of the foot where no corrections or balancing is performed, but additional support is added to enhance or relieve pressure to the transverse metatarsal arch and the longitudinal arch.

How To Make Impressions:

FOAMART ® IMPRESSION SYSTEMS

After careful examination of the foot, mark all areas that will require modifications with an indelible pencil. This will be transferred to the FOAMART® impression to provide us with more information. In addition, you may draw a line bisecting the calcaneus so that we will have a reference line called 'Neutral'.

•  Seat the patient. Establish a 90° angle at the knee. Be sure that the patient is seated far forward on the chair so that pressure can be exerted freely above the knee to assure a deep impression with control.

•  Instruct the patient to relax and allow you to apply even pressure. The patient should be instructed NOT TO ASSIST in exerting pressure.

•  Hold the lateral and medial malleolus assuring a neutral positioning of the Talus and exert pressure from above the knee. Press heel deeply into the FOAMART®.

•  The depth of the FOAMART® impression should be governed by the height of the longitudinal arch. Covering this important area will assure an accurate 'positive' cast. Press the foot down to the bottom of the box.

•  Maintain the limb at 90°. Press the instep down into the FOAMART® with uniform pressure.

•  Complete foot impression by pressing on the Dorsum of the toes, plantar flexing the toes into the FOAMART® material down to the bottom of the box.

•  If there are any gaps between the foot and FOAMART® impression due to movement of the foot while located in the foam, you may press the foam "inward" towards the foot. This will close the gaps and insure a more accurate cast and better contact in our orthosis.

•  Remove the patient's foot from the FOAMART® impression without distorting the material and repeat with the other foot. If indelible pencil was used to mark specific areas, please go over these marks again to insure that they will be accurately transferred to the positive cast.
